Before your first appointment, and periodically throughout your care, you’ll complete a few short questionnaires. You’ll also have opportunities to share ongoing feedback about your sessions. These tools allow us to:
- Better understand how you’re feeling about your care.
- Track how your work with your dietitian is progressing.
- Adjust and tailor support in real time to meet your specific needs.
Your responses are confidential and used solely to enhance your care. Most questionnaires include 4–16 quick-tap questions and take only a few minutes on your phone, tablet, or computer.
